Tell the front desk at either clinic, or call and ask for our civil rights coordinator. You can ask for this notice in another format or language the same way.

If you think we treated you unfairly
You can file a complaint with our civil rights coordinator in person, by phone or by mail, and we will help you file it if you need.
You can also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, online, by phone at 1-800-368-1019 (TDD 1-800-537-7697), or by mail at 200 Independence Avenue SW, Room 509F, HHH Building, Washington, DC 20201.
